
Instructions for Medical Record Requests
​Ocala Family Medical Center has partnered with HealthMark Group to make requesting your medical records simple and easy! As our release of information partner, HealthMark is here to streamline the process and get you your records faster and more efficiently.

How do you request records?
Requests are submitted electronically through HealthMark’s Request Manager at https://requestmanager.healthmark-group.com. You will first need to enter the email address you want the records sent to when the request has been completed. Within a few minutes, you will receive an email to begin your records request submission. Once logged in, select “Submit Request” from the menu options and enter all required fields to provide an authorization directly to HealthMark. Once your medical records request is completed, a notification will be sent via email that your records are available for download.
*Outside provider offices, attorney offices, etc. will follow the same process as above. Use YOUR email address to have the confirmation/download email for the records sent to and use the PATIENT’S information on the request form.
IMAGING REQUESTS – Imaging requests can now be made through the HealthMark request dashboard. Once your request is processed, you will receive an email from HealthMark that your request has been completed, as well as an email from Powershare (our imaging hosting platform) to login and access the images you requested.
How long does it take to process your requests?
Your record requests are generally processed within 8 business hours. What does that mean? One or two business days after you submit your request, your records will be delivered electronically – right to your inbox!
Do you have to remember another username and password?
Nope! HealthMark’s Request Manager does not require usernames or passwords for logging in. Simple email verification and secure links do the trick – getting you your records quickly, efficiently and securely – no password required.
